| IMPORTANT: There will be only 2 different forms; for Six Man Team Regionals or all other individual tournaments. These forms are in a word document so you will be able to save to your computer when obtaining information from others and also in pdf for printing. Please note, we expect anglers to type their information on the form, WE NEED TO BE ABLE TO READ THE INFORMATION. For PDF, please print legibly. If we can’t read it, please don’t complain about your name being misspelled. WE NEED CELL PHONE NUMBERS! For Team Regional 1, mail applications to the State Tournament Director. For Team Regional 2, they need to be mailed directly to the hosting district not the state tournament director. For individual tournaments send to your district tournament director. For Mr Bass Kickoff and the State Championship send to the State Tournament Director. See the tournament page for details. Any apps without links will be provided as soon as possible. If you HAVE NO INTENTION of going to the championship, please check the box indicating this on your entry form. |
